In-Home Skilled Nursing in Georgia
365 Health Care provides personalized in-home skilled nursing for all ages—children, adults, and seniors. Licensed nurses support medical needs according to the client’s care plan, provider orders, and the services arranged with our team.
Nursing care where you feel at home
In-home nursing brings professional medical support into a familiar environment. Care is tailored to the person’s condition, daily routine, and ordered care plan. Families can discuss ongoing medical needs, support following a change in health, or nursing needs alongside personal-care assistance.
What your nursing plan may include
Depending on provider orders, clinical assessment, nurse qualifications, and service availability, a plan may address monitoring health changes, ordered medication-related care, support with prescribed treatments, and teaching families about established care routines. Specialized needs must be reviewed by our clinical team before services are confirmed. We do not assume that every medical task is available for every client.
Care for children, adults, and seniors
Our skilled nursing service is available to all age groups. Pediatric care can be discussed alongside GAPP eligibility for children ages 0–20. Adults and seniors can explore arrangements based on their care needs and payment options. Each plan is individual; GAPP age limits do not apply to our general private-pay skilled nursing service.
Working with the care plan
Nurses follow the authorized care plan and applicable provider orders. Families share relevant care information and discuss routines, concerns, and changes with the team. Personal-care assistance is not a substitute for licensed nursing, and nursing services should not be confused with the Parent Pay Option.
Discuss services and payment
Contact us to review the care needed, preferred schedule, location, and payment arrangement. Eligible children may qualify for GAPP nursing services; families of all ages can discuss private-pay care. Service availability, hours, and costs are confirmed before care begins.
